Summer Camp Season of 2010

Dear Camper:

Another year has flown by and it is once again time to start thinking about summer camp. Last year
there were about 740 individuals who came to camp. There were tons of smiles, a few tears, and
thousands of memories for everyone. This year is shaping up to be every bit as wonderful as last. Camp
is a place where amazing things happen.

If the camper has a Public Guardian, please make sure that his/her Guardian forwards the Public
Guardian Participation Agreement to camp as soon as possible.

The medical form will be sent to you in the Acceptance Package via E-mail. Once you receive the Email
print the Medical Form and take it to your doctor to be completed and signed. I need to receive
the medical form no later than 14 days before the start of your camp or you risk having your
application withdrawn. Please ensure that your doctor has signed and dated the form as well as filled
out every question. Note: ***When you send in your Medical Form, please include the
date(s) of the camp(s) you are attending on it in the upper right hand corner.***

Please remember that Camp Health, Hope & Happiness requires a minimum $100.00 deposit in order
to process your Summer Camp Application (full camper fee payments are accepted with the application).
The $100.00 Deposit will be applied towards your camper fee. The deposit is non-refundable unless
you meet our cancellation policy guidelines. Your full camper fee, as well as any outstanding
documents, need to be received by camp no later than 14 days prior to the first day of the scheduled
camp otherwise you risk having your application withdrawn.

Please Note:
• The $100.00 deposit can be paid by Mastercard, Visa or Cheque. If you chose to pay by
cheque it needs to be received at camp NO LATER THAN 10 DAYS AFTER THE DATE
OF REGISTRATION
or your Camper Application will be terminated
• If you have special circumstances please contact the office by phone or e-mail.

Please make sure that you have viewed our Screening Criteria and Refund /
Cancellation Policy BEFORE completing an Online Camper Application.
